How to File a Claim
To file a claim under an UZIN Warranty, you must contact us, in writing, within twenty (20) days of the discovery of the alleged breach of this Warranty, directed to the following:

The following items must be provided at the time the claim is submitted:
- Description of the alleged breach, proof of purchases(s) for UZIN products, floor covering specifications, date and site of installation, and documentation of initial pre-installation test res ults for dewpoint and substrate moisture and pH testing (if relevant to the circumstance).
- Core samples must also be taken and analysis of same provided in the event such is needed in order to resolve the claim.
If we determine that the alleged defect is covered by our Warranty, we will reimburse you for the costs of the core sample(s) and attendant analysis.
Additionally, the owner of the jobsite must consent to total access to the site for inspection purposes if needed, and you must make yourself accessible and cooperate with UUNA as required in order for us to evaluate the claim.
Any repair work performed before the claim is received and evaluated will render this Warranty null and void.
